The Venezuelan health reform Barrio Adentro (BA), initiated in 2003, aims to improve population health and promote popular participation. Access to healthcare has improved but the distinguishing feature of BA is participation by communities. Popular participation has been institutionalized through the formation of health committees and communal councils. We explored the challenges/barriers faced by these community groups in BA. A mixed-methods participatory methodology was used involving 31 participants selected through non-random purposive sampling from five Venezuelan states. Results indicate that implementation issues challenge participation. There is political will at the highest level but the opposition political parties, Venezuelan Medical Federation, and the private media oppose the reform. Public participation in state affairs is facilitated if there are popular arenas of participation that promote engagement of organized citizenry with the state and an ongoing interaction between state and civil society in order to radically transform the state La reforma de salud venezolana Barrio Adentro (BA), iniciada en 2003, intenta mejorar la salud de la población y promover una participación popular. Se ha mejorado el acceso al cuidado de la salud, pero la característica de BA es la participación por comunidades. La participación popular se ha institucionalizado a través de la formación de comités de salud y concejos comunales. Hemos explorado los retos/barreras enfrentados por esos grupos comunales en BA. Se usó una metodología participativa de métodos mixtos, involucrando treinta y un participantes seleccionados, a través de un muestreo intencional de cinco estados venezolanos. Los resultados indican que la implementación de los pormenores dificulta la participación. Hay voluntad política al nivel más alto, pero los partidos políticos de oposición, la Federación Médica Venezolana y los medios privados, se oponen a la reforma. La participación pública en asuntos de estado se facilita si existen arenas populares de participación, que promuevan la colaboración de ciudadanos organizados con el estado y una interacción continua entre el estado y la sociedad civil, para transformar radicalmente al estado. 委内瑞拉的“全国健康计划”医疗改革始于2003年，旨在改善公民健康状况，促进公众参与。“全国健康计划”便利了医疗卫生服务的获取，但其显著特征是社会的参与。通过组建卫生委员会和社区委员会，公众的参与得以制度化。本文探讨了“全国健康计划”中的这些社区团体所面临的挑战或障碍。通过对委内瑞拉5个州进行非随机目的抽样，本文选取了31位参与者，使用了包含多种方式在内的参与式研究方法。结果表明，医疗改革的落实问题阻碍了民众的参与。尽管最高层存在政治意愿，但反对党、委内瑞拉医药联合会和私营媒体反对这项改革。公众参与平台的存在将便利公众参与国家事务。这些平台将促进有组织的公民参与国家事务，并推动当前国家与公民社会之间的互动，它以实现国家重大转变为目的。 2003년에 도입된 베네수엘라의 의료개혁인 Barrio Adentro(BA)는 전체 인구 건강을 증진시키고 대중적 참여를 촉진시키려는 목적을 가지고 있다. 의료보호를 이용하는 것이 개선되었지만, BA의 뚜렷한 모습은 커뮤니티 참여이다. 대중적 참여가 건강 커뮤니티와 마을 위원회 구성을 통해서 제도화되었다. 우리는 BA에서 이들 커뮤니티 집단이 직면하고 있는 도전과 장애를 다룬다. 5개 베네수엘라 지역에서 비무작위적으로 표집된 31명의 참여자들을 대상으로 혼합연구방법(mixed methods)이 적용되었다. 결과는 집행이 참여에 있어서 문제가 된다는 점을 보여준다. 최상층에서 정치적 의지가 있지만, 야당들, 베네수엘라 의료협회와 민영 매체들이 개혁을 반대한다. 국가를 급진적으로 변혁하기 위하여 조직된 시민들이 국가에 관여하는 것과 국가와 시민사회 간의 지속적인 상호작용을 촉진시키는 참여의 영역이 있다면, 국가 업무에 공적인 참여가 촉진된다.
