Inspiratory Concentrations of O2, N2 and N2O, Arterial Oxygenation and Acid-Base Status during Closed System Halothane Anaesthesia in the Horse1

Summary In 40 horses anaesthetized with halothane in a closed circuit apparatus the anaesthetic bag was filled with 5 per cent halothane, carried by an oxygen/nitrous oxide ratio of 1/2 or 1/3 after flushing the circuit with 50 litres of oxygen. After connection of the horses to the apparatus the nitrous oxide was switched off and a continuous flow of 2 litres of oxygen was used. This technique gave high inspiratory oxygen-concentrations. These inspired O2 concentrations always sufficed to obtain a satisfactory arterial oxygen tension in healthy horses in lateral recumbency and in young horses in the supine position. In older and especially heavy horses in the supine position the arterial oxygen tension proved insufficient even with very high inspired oxygen concentrations. Zusammenfassung Die Konzentration von O2, N2 und N2O in der Inspirationsluft; Oxygenierung und Säure-Basen-Verhältnis im arteriellen Blut während der Halothan-Narkose im geschlossenen System beim Pferd 40 Pferde wurden im geschlossenen System mit Halothan narkotisiert. Nach dem Durchwaschen des Systems mit 50 Litern Sauerstoff wurde der Atembeutel mit 5 Volum-% Halothan in einer Mischung von Sauerstoff und Lachgas im Verhältnis 1:2 oder 1:3 gefüllt. Nachdem das Pferd an das Gerät angeschlossen worden war, stellte man den Zustrom von Lachgas ab und ließ kontinuierlich pro Minute 2 Liter Sauerstoff in das System fließen. Diese Methode ergab eine hohe O2-Konzentration in der Inhalationsluft. Die so eingeatmete Sauerstoffmenge war genügend, um bei gesunden Pferden in Seitenlage und bei jungen Tieren in Rückenlage eine ausreichende O2-Spannung im arteriellen Blut zu erhalten. Bei älteren und besonders bei schweren Pferden in Rückenlage war die arterielle O2-Spannung trotz sehr hoher O2-Konzentration in der Inspirationsluft ungenügend. Après un lavage du système par 50 litres d'oxygène, on remplit la poche respiratoire avec 5 volumes % d'halothane dans un mélange oxygène-gaz hilarant 1:2 ou 1:3. Lorsque le cheval est relié au circuit, on arrête l'aflux de gaz hilarant et on maintient un débit continu d'oxygène de 2 litres/minute. On obtient par cette méthode une concentration élevée en oxygène dans l'air d'inspiration. Cette quantité d'oxygène inhalée est suffisante pour obtenir chez les cheveaux sains en décubitus latéral et chez les jeunes animaux en décubitus dorsal une tension d'oxygène convenable dans le sang artériel. Chez les chevaux plus âgés et en particulier chez les chevaux lourds en décubitus dorsal, la tension d'oxygène artérielle reste insuffisante, malgré la très forte concentration d'oxygène de l'air inspiré. Tras lavar el circuito con 50 litros de oxígeno, se llenó la bolsa respiratoria con 5 volúmenes % de halothane en una mezcla de oxígeno y protóxido de nitrógeno en proporción de 1:2 ó 1:3. Esta técnica logró una concentración elevada de O2 en el aire inspiratorio. La concentración de oxígeno respirada así era suficiente para obtener en caballos sanos en decúbito costal y en animales jóvenes en decúbito supino una tensión de O2 satisfactoria en la sangre arterial. En caballos de edad avanzada y sobre todo en los pesados en decúbito supino, la tensión arterial de O2 resultó insuficiente en el aire inspiratorio a pesar de la concentración de O2 muy elevada.
