Russian National Registry of IPF patients was run in 2016 and includes 1027 patients to Feb, 2019. We present baseline characteristics of IPF patients in Russia. <b>Aim:</b> To characterize demographic, clinical, and physiological parameters of IPF patients in Russia. <b>Methods:</b> This was a prospective observational real-life clinical study. Patients were enrolled in the Russian National IPF Registry in 71 regions of Russia. Baseline data included demographic, clinical, functional, radiological and histological data. <b>Results:</b> To the end of 2018, the Russian Registry involved 978 IPF patients; mean age, 64.6±10.8 yrs; 562 males (57.5 %), 374 (38.2 %) were smokers or ex-smokers; 272 (27.8%) had definite IPF. The length of the disease was 19.8±28.4 months. Restrictive ventilation disorders were found in 70.6 % of patients; DLCO decrease < 80% pred. was found in 97.0 % of patients. Surgical lung biopsy was done in 164 patients. Definite of probable IPF was diagnosed in 27.8% of patients. Sixty three patients (6.4%) died during 2-yr follow-up. At baseline, 41% and 15% of patients were treated with steroids and high-dose NAC, respectively; 14% were prescribed antifibrotic agents. In 2 years of the follow-up, steroid prescription rate decreased to 31.5%, antifibrotics prescription rate increased to 19%. <b>Conclusion:</b> The Russian Registry of IPF patients provides useful information on IPF prevalence, demographic and clinical features of IPF course in Russian patients and in physicians’ knowledge of diagnosis and treatment of this disease.
