Real-world outcomes for incidental interstitial lung abnormalities diagnosed over the baseline round of the Yorkshire Lung Screening Trial.

<b>Introduction:</b> Interstitial lung abnormalities (ILA) are a common incidental finding on low-dose computed tomography (LDCT) scans from participants in lung cancer screening (LCS) programmes. Few studies have examined the downstream impact of these findings, specifically how many patients receive pharmacotherapy. <b>Method:</b> Baseline screening LDCT reports for participants in the Yorkshire Lung Screening Trial were reviewed for mention of ILA. Cases flagged as clinically significant were reviewed by a Thoracic Radiologist and Respiratory Physician in a Screening Review Meeting (SRM) and referred to the Interstitial Lung Disease (ILD) service if appropriate. Healthcare records were retrospectively reviewed for all cases of ILA and clinical outcomes recorded. <b>Results:</b> A flow chart for all participants is shown in Figure 1. In total, 6,650 participants underwent baseline LDCT of whom 169 (2.5%) had a reported incidental finding of ILA. Of these, 153 were reviewed in SRM and 55 patients were referred to the ILD service. Five patients commenced anti-fibrotic treatment for IPF and 3 commenced Prednisolone for non-IPF diagnoses; an overall treatment rate of 0.12% for those undergoing LCS. 31 patients (0.5%) are in active monitoring. <b>Conclusion:</b> LCS will detect a small number of patients with significant, unrecognised ILD who qualify for pharmacotherapy. <b>Funding:</b> Yorkshire Cancer Research (L403)
