P552: Bringing equitable access to cancer genetic services: Implementing and adapting a digital hereditary cancer screening tool at an FQHC

Hereditary cancer risk assessment (HCRA) is a key avenue for primary prevention that enables targeted interventions and surveillance strategies by identifying individuals at increased risk for cancer. Despite longstanding evidence-based guidelines for implementing HCRA, efforts to do this outside of oncology or medical genetics remain limited. Disparities in family history collection drives further disparities in risk identification. Current precision prevention strategies have the potential to worsen disparities in cancer outcomes if not implemented equitably.
