Real-world effectiveness of mepolizumab in patients with severe asthma with/without chronic rhinosinusitis with nasal polyps: pooled analysis from European cohorts

<bold>Background:</bold> Patients with severe asthma and an eosinophilic phenotype (SAEP) with comorbid chronic rhinosinusitis with nasal polyps (CRSwNP) typically have high NP recurrence rates and poor asthma control. There remains limited evidence on the effectiveness of mepolizumab in patients with SAEP with/without CRSwNP (SAEP +NP/SAEP -NP, respectively) in clinical practice. <bold>Aim:</bold> To compare the characteristics and clinical outcomes of patients with SAEP +NP and SAEP -NP, treated with mepolizumab, from nine countries. <bold>Methods:</bold> Demographic characteristics and clinical patient data from REALITI-A, REDES and RELIGHT studies were described 12 months pre versus post mepolizumab initiation, among adults with SAEP +NP and SAEP -NP. The change in the annual rate ratio (aRR) of clinically significant asthma exacerbations (CSEs) was assessed. <bold>Results:</bold> Among 867 patients (Table), the aRR (95% CI) of CSEs decreased significantly post mepolizumab initiation in patients with SAEP -NP (0.28 [0.25–0.31]) and SAEP +NP (0.19 [0.17–0.23]). In both cohorts, especially the SAEP +NP cohort, there was a significant improvement in lung function and symptom control, and a significant decrease in blood eosinophil count versus baseline. <bold>Conclusions:</bold> These data corroborate the real-world effectiveness of mepolizumab across Europe in patients with SAEP, with or without comorbid CRSwNP. <fig><object-id>erj;64/suppl_68/PA3929/F1</object-id><object-id>F1</object-id><object-id>F1</object-id><graphic></graphic></fig>
