Primary progressive aphasia in the neurologist practice

The article presents data on the epidemiology of stroke and dementia in the Moscow region of the Russian Federation, Western Europe, and the United States. A brief review of the literature on the problem of primary progressive aphasia (PPA) is provided, and the criteria for diagnosis and the main variants of the disease are described. Two clinical cases are presented: a patient with an agrammatic form of PPA and a patient with post-stroke aphasia. The findings of routine MRI, functional MRI with a speech paradigm and diffusion tensor imaging with tractography are shown, and the main differences in the presentation and the MRI findings in patients with similar clinical presentation of motor aphasia caused by the stroke-related neurodegenerative process are analyzed. Additional tests are provided for early detection of the agrammatical form of PPA.
