638 Improving Ureteric Stent Management: Implementing a Computer-Based Stent Register to Enhance Patient Safety

Abstract Aim To assess the ureteric stent register within our department, ensuring its proper maintenance to avoid missed stents and their associated complications. This project aims to implement a unified, computer-based stent registry with automated reminders to enhance patient safety and minimize medicolegal risks. Method We conducted a retrospective audit of ureteric stenting practices in our department, analyzing data from elective and emergency stenting procedures performed over two months (June and July). Data collection was done via a manual review of theatre registers and a list provided by the booking team. Microsoft Excel was utilized for data analysis and visualization. In the first audit cycle, 44 patients were included, with 36 having stents inserted. Of these, seven were documented in the stent register, while 29 were not. The mean interval for stent removal was 3.5 weeks (range: 1 to 12 weeks). Thirteen stents remained in situ without documented removal. Results Our findings highlighted significant gaps in stent register documentation, with only a minority of stents being appropriately recorded. Despite these deficiencies, stent removal was achieved for most cases within the expected timeframe. However, missed stents, including two forgotten for over five years, underscored the need for a robust tracking system. The introduction of an electronic stent register linked to patient records and automated reminders is currently being implemented and will be evaluated in the second audit cycle. Conclusions The absence of a reliable stent registry presents risks to patient safety and increases medicolegal liabilities. A computer-based system holds promise for reducing missed stents and improving compliance with follow-up protocols.
