<h3></h3> Since the publication in 2023 of Dr. Mina Johanssen’s article highlighting the importance of considering the time it takes to implement recommendations in practice, the concept of Time Needed to Treat (TNT) has garnered significant interest. We propose using this concept to highlight an important aspect of low-value care and overuse: the opportunity cost. While it is logical to consider the time we spend on the care we provide, clinicians often struggle to estimate how much time various clinical tasks take in their practice. It originated when a guideline group (CTFPHC) compared two sets of recommendations on the prevention of fragility fractures (their own and another) using the TNT concept. This group used a survey to estimate the time required for various actions needed to implement care (e.g., ordering tests, interpreting results). When we began drafting a document to help clinicians navigate recommendations for fatty liver disease, we conducted a similar exercise and were struck by the wide range of time estimates clinicians provided. This realization led to the idea of developing a calculator. Our tool allows clinicians to input their own values, customizing calculations to their specific practice contexts and fostering reflection on their practice. The development process involved consultation with colleagues and diverse experts to ensure clinical relevance and usability. The tool underwent iterative testing to refine its features and functionality. Feedback was used to make adjustments and enhancements, ultimately creating a customizable and practical calculator. We believe this tool will be invaluable in facilitating task prioritization and the deimplementation of low-value care. Participants in this seminar will explore the concept of TNT, learn about our TNT calculator, and discuss its application in various contexts provided by the authors. This session will serve as a critical step in launching the calculator.
