Abstract: Introduction: Competitive stress, irregular schedules, and circadian misalignment create unique sleep disruptions among elite performers that traditional sleep hygiene recommendations insufficiently address. We propose reframing sleep from passive recovery to active brain-based performance medicine – a proactive approach to optimizing brain and body function. Framework Development: We integrated evidence from sleep science, sports medicine, and neuroscience, with clinical observations from sports psychiatry practice across professional leagues and high-stakes settings, with a focus on sleep’s role in stress hormone regulation, brain waste clearance, muscle recovery, circadian entrainment, and energy system management (the regulation of the body’s energy production and use). Clinical insights were derived from treatment protocols developed while working with athletes in major professional leagues and executive populations in high-stakes environments. Framework Summary: Key modifiable systems emerged as primary improvement targets: stress hormone regulation, brain waste clearance mechanisms, circadian entrainment and energy systems management. Despite achieving lower average sleep efficiency than controls (80.6% in Olympic athletes vs. 88.7% in matched non-athletes), elite performers maintain high outcomes by strategically optimizing these systems. Notably, Olympic athletes exhibit far greater variability in sleep parameters compared to controls. Discussion: Sleep improvement functions as performance medicine, offering measurable advantages in cognitive function, stress resilience, and career longevity. When integrated with energy management strategies, this framework supports performance enhancement. However, consumer sleep tracking technology poses clinical challenges, with “sleep score” fixation potentially undermining optimization benefits. Conclusion: A systems-based, personalized approach reframes sleep as a performance intervention rather than a universal prescription. Tailoring strategies to body clocks and competition demands offers a practical pathway toward both athletic excellence and long-term career sustainability. [Box: see text]
