(185) Retrospective Analysis of Satisfaction in Patients Undergoing a Virgin Insertion of Penile Prosthesis or a Removal and Placement of Inflatable Penile Prosthesis for Mechanical Failure in the Predominantly Spanish Speaking Latin Patient Population

Abstract Introduction Inflatable penile prosthesis (IPP) placement is a definitive treatment for erectile dysfunction that improves quality of life (QOL) and sexual satisfaction. However, patient-reported outcomes (PROs) may be influenced by cultural and linguistic factors, which are not well studied in Spanish-speaking populations. Historically, PRO instruments such as the Satisfaction Survey for Inflatable Penile Implant (SSIPI) and Quality of Life and Sexuality with Penile Prosthesis (QOLSPP) have been validated only in English or Italian, limiting their use in Spanish-speaking patients. Anecdotal clinical experience suggests that predominantly Spanish-speaking Latino patients report higher satisfaction post-IPP than their English-speaking counterparts. This study evaluates the differences in post-operative QOL and satisfaction in Spanish vs. English-speaking patients, and explores whether cultural relevance of sexual function contributes to these outcomes. Objective To retrospectively compare post-operative QOL and satisfaction, following virgin placement of IPP or removal and placement of IPP for mechanical failure, between Spanish-speaking and English-speaking men using SSIPI and QOLSPP scores. Methods This retrospective cohort study included patients who underwent virgin IPP placement or removal and replacement of end-of-life devices between 2020 and 2025 at two high-volume prosthetic centers. IPP placement occurred within 6–60 months prior to the study. Patients were stratified by self-reported preferred language (Spanish vs. English). All participants completed the SSIPI and QOLSPP surveys in their native language. Exclusion criteria included non-compliance with questionnaires or surgeries outside the defined window. A total of 150 men will be enrolled; 139 men have completed enrollment to date. Demographic and clinical characteristics were collected, including age, relationship status, ethnicity, and sexual orientation. Results Of the 139 patients analyzed, 64 were Spanish-speaking and 75 were English-speaking. The groups were similar in age, and partner status. Spanish-speaking patients reported significantly higher SSIPI total scores (mean: 75.76 vs. 68.6; p < 0.05), particularly in domains related to sexual satisfaction and happiness with the device. Spanish-speaking patients also scored higher on the QOLSPP (mean: 72.1 vs. 65.7 (p < 0.01). Conclusions In our preliminary analysis, Spanish-speaking patients report greater satisfaction and quality of life following IPP placement compared to English-speaking patients. These findings suggest that sexual function may carry greater cultural significance in Spanish-speaking populations, potentially enhancing perceived benefit from surgical intervention. Clinicians should consider cultural and linguistic context when counseling patients undergoing IPP to improve patient-centered care and satisfaction outcomes.
