10038-NQPC-1 Practice of intervention in comprehensive treatment of malignant glioma through a collaborative model utilizing local medical resources and expertise

Abstract The disease of brain tumors, including malignant gliomas, results in a decline in physical and cognitive functions, infections and skin problems associated with weakened immunity, and loss of social status, all of which require multifaceted support in medical and welfare services. As the disease progresses, neurological dropout symptoms and cognitive decline occur, and the amount of caregiving inevitably increases, placing a heavy burden on family members. There are a certain number of cases in which key persons and caregivers are absent, and the utilization of local nursing and medical resources is essential not only for the terminal stage of the disease, but also for the continuation of multidisciplinary treatment. After 18 years of service at a flagship hospital, I currently provide outpatient and home-visit care in the community, specializing in cranial nerve diseases including malignant glioma, in collaboration with the hospital and other flagship hospitals in the region. I am also involved in the core hospital as a part-time doctor, collaborating with the ward staff and full-time doctors. This collaboration between hospital and home care by the same doctor enables seamless information sharing, accurate understanding of medical conditions, and provision of specialized advice and treatment, and is thought to lead to increased peace of mind for patients and their families and contribute to trust on the part of the core hospital. The home-visit clinic specializing in cranial nerve diseases is an important support system that contributes to the continuation of multidisciplinary treatment as well as the terminal stage. In the future, further dissemination of a collaborative model that takes advantage of local medical resources and expertise will be required. In this presentation, we will report on the practice of visiting doctor specializing in cranial nerve diseases.
