Easy Read: Inclusion Fails by Design: A Taxonomy of Harm in Systems

This work presents a visual and conceptual taxonomy of how organisational systems predictably produce harm while claiming inclusion, care, or psychological safety. Rather than treating burnout, disengagement, silence, or conflict as interpersonal or cultural failures, the taxonomy identifies recurring structural design patterns through which harm is absorbed, obscured, disciplined, or extracted within institutions. The diagrams map four primary mechanisms: absorptive system design performative inclusion punitive ambiguity extractive inclusion These patterns are shown not as ethical lapses or leadership shortcomings, but as predictable outcomes of system architecture, incentive structures, and power arrangements. The work is intended as a diagnostic and design tool for practitioners, researchers, advocates, and organisational leaders working in inclusion, safeguarding, disability justice, and systems change. It is designed to be used as a reference artifact, not a motivational resource. This work is a conceptual framework presented primarily through visual system diagrams. It is intended to support analysis, teaching, and organisational design conversations rather than empirical measurement.
