Concomitant Occurrence of Right Atrial Myxoma and Coronary Artery Disease: An Unusual Clinical Presentation

Abstract Primary cardiac tumors are rare, with the autopsy incidence of 0.001%–0.03% and secondary cardiac tumors are more common with the autopsy incidence of 1.7%–14%. Primary cardiac tumors are mostly benign and myxomas are the most common among it. Left atrial myxoma with coronary artery disease (CAD) is more common than right atrial (RA) myxoma. Hereby, we present the rare case of right atrial myxoma with CAD. A 55-year-old hypertensive and hypothyroid female, nondiabetic with no prior ischemic heart disease, experienced exertional dyspnea, NYHA class II, for a year, progressing to class III recently. Clinical examination and systemic examinations were unremarkable. The electrocardiogram revealed inverted T-waves in leads V1-6. Transthoracic echocardiography identified large mobile mass on the interatrial septum. Coronary angiography revealed an 80% stenosis in the proximal right coronary artery (RCA), with normal other coronaries. The patient underwent myxoma excision and a saphenous vein graft to RCA.
