AI-driven facial scanning for contactless monitoring of vital cardiovascular parameters: validation of the vuearo mirror system against standard cuff-based devices

Abstract Background Accurate and continuous monitoring of blood pressure (BP) and heart rate (HR) is critical for timely cardiovascular risk assessment and intervention. The VueAro Mirror is an AI-powered, contactless facial scanning system designed to estimate vital cardiac parameters in real time. This study evaluates the performance of the VueAro system against a clinically validated automated cuff-based BP monitor, with subgroup analysis across age and gender. Methods In a prospective study involving 769 adults (aged 18–80), simultaneous measurements of systolic BP, diastolic BP, and HR were obtained using both the VueAro system and a standard automated BP device. Agreement between methods was assessed using paired t-tests, Bland-Altman analysis, and stratified error visualizations. Subgroup analyses examined algorithmic performance variation by age and gender. Results Statistically significant differences were found between the VueAro and cuff-based device readings for all parameters (systolic BP, diastolic BP, and HR; all p < 0.05). The VueAro system slightly underestimated systolic BP and overestimated diastolic BP and HR. Measurement deviations were influenced by demographic factors, suggesting algorithm tuning opportunities. Bland-Altman plots revealed consistent trends with acceptable limits of agreement, supporting foundational validity with some systematic bias. Conclusions The VueAro Mirror demonstrates strong potential as a non-invasive, AI-powered cardiovascular monitoring tool. While initial differences from standard cuff-based devices exist, they are within a range that could be improved with demographic-specific model calibration, integration of PPG signal data, and adaptive signal processing techniques. These findings underscore the feasibility of AI-enhanced facial scanning for scalable, contactless cardiac health monitoring, aligning with the broader vision of remote digital health solutions in cardiology.
