Managing Medication Optimization in Managing Complex and Chronic Conditions: A Multidisciplinary, Patient-Centered Approach

Medication optimization is a critical component of effective management for patients living with complex and chronic conditions, including heart failure, asthma, diabetes, and other high-burden diseases. These conditions frequently require long-term, multi-drug regimens that increase the risk of polypharmacy, adverse drug events, and poor adherence. This narrative review synthesizes evidence from international clinical guidelines, randomized controlled trials, and real-world studies published between 2010 and 2024 to examine the principles, disease-specific strategies, and clinical outcomes associated with medication optimization. Findings consistently demonstrate that structured, patient-centered, and multidisciplinary optimization approaches improve clinical outcomes, reduce hospitalizations, and enhance quality of life. The review further highlights persistent implementation gaps and discusses emerging technology-enabled and pharmacist-led models that support sustainable optimization in clinical practice.
