P0208 Combining SUS-CD and Sonographic Signs for Accurate Localization-Based Assessment of Inflammatory bowel diseases Inflammation

Abstract Background Inflammatory bowel diseases (IBD) are chronic conditions that require an accurate assessment of inflammatory activity to tailor treatment. The SUS-CD scale provides a quantitative assessment of the inflammatory process using ultrasound data. Aim: To evaluate the diagnostic value of the SUS-CD scale as an integral ultrasound tool for assessing inflammatory activity in IBD, with an emphasis on the influence of the anatomical localization of inflammation and associated ultrasound signs. Methods The study included data from 72 patients with IBD (ulcerative colitis (UC, n = 40), Crohn’s disease (CD, n = 32). A comparative analysis of the mean SUS-CD score was performed depending on the location of inflammation, the presence of intestinal wall changes, and mesenteric fat and lymph node involvement. Analysis of variance and regression analysis, including factor interactions, were used. Results Statistically significant differences in SUS-CD values were found between inflammatory sites (p < 0.05). Wall involvement and the presence of lymph nodes positively correlated with high inflammatory activity. Extended regression models confirmed the significance of the interaction between anatomical and inflammatory parameters. The SUS-CD and ultrasound findings by site are presented in Table 1. The frequency of ultrasound findings by site (%) is shown in Figure 1. The analysis confirms the significant role of anatomical location in determining the degree of inflammation. Ileal involvement is associated with the highest SUS-CD scores, which correlates with a 70% incidence of wall thickening and 65% of mesenteric changes. This underscores the pathophysiological importance of this location in Crohn’s disease. High activity values are also noted in the descendens region, requiring clinical attention. Conclusion Mesenteric lesions and lymph nodes are more common with high SUS-CD values, highlighting their role as additional ultrasound markers of inflammation. Combining visual features with the SUS-CD scale enhances diagnostic accuracy and may be useful for dynamic therapy monitoring.
