Systemic Physiological Dysregulation After Musculoskeletal Trauma: Implications for Recovery and Orthopedic Decision-Making

Musculoskeletal trauma has traditionally been approached as a localized structural injury; however, accumulating evidence demonstrates that it initiates complex systemic physiological responses that critically influence recovery. Inflammatory activation, immune dysregulation, endothelial dysfunction, and metabolic–mitochondrial stress interact dynamically over time, shaping clinical trajectories beyond fracture stabilization alone. This integrative review synthesizes current evidence on systemic physiological responses following musculoskeletal trauma and examines their relationship with recovery-related outcomes, including complications, organ dysfunction, and functional rehabilitation. Peer-reviewed literature from orthopedic trauma, critical care, immunology, and trauma physiology was analyzed thematically using a process-based framework that considers temporal phases of injury and recovery. The reviewed studies consistently describe an early hyperinflammatory phase associated with injury severity and physiological instability, followed by progressive immune dysfunction, endothelial alterations, and sustained metabolic stress. These systemic patterns are linked to time-dependent risks such as multiple organ failure, infection susceptibility, and delayed recovery. Surgical timing emerges as a relevant modifier of systemic burden, supporting physiology-driven strategies such as damage control orthopaedics during vulnerable periods. Overall, recovery after musculoskeletal trauma is strongly influenced by systemic physiological trajectories rather than by local injury characteristics alone. Recognizing trauma as a dynamic, whole-body process provides a framework for more individualized orthopedic decision-making and may contribute to improved recovery outcomes.
