Rototiller-related degloving injury of the foot managed with staged debridement, negative pressure wound therapy, and one-stage Matriderm® and split-thickness skin grafting

Rototiller injuries are associated with severe contaminated soft-tissue trauma and a high risk of delayed necrosis, particularly in patients with diabetes mellitus. We present a case of a 71-year-old man with type 2 diabetes who sustained a complex dorsolateral degloving injury of the left foot with exposed tendons, bones, and joints. Following staged debridement, infection control with negative pressure wound therapy (NPWT), and definitive reconstruction using a dermal substitute (MatriDerm®) combined with split-thickness skin grafting (STSG), excellent functional and aesthetic outcomes were achieved.
