La consultation d’anesthésie est une étape clé pour dépister les patients allergiques. Si beaucoup de ces patients déclarent être allergiques à un ou plusieurs médicaments, ces allergies ne sont pas toujours avérées. Les antibiotiques, les anti-inflammatoires non stéroïdiens et les opioïdes sont fréquemment cités par les patients, mais ils ne sont pas nécessairement les principaux responsables de réactions périopératoires. Certaines situations sont parfois interprétées à tort comme des allergies (antécédents familiaux, tolérance ultérieure au même médicament, signes cliniques non évocateurs), et dans ces cas, le diagnostic peut être facilement infirmé. Dans les autres cas, l’histoire de la maladie seule est souvent insuffisante pour poser le diagnostic d’une allergie médicamenteuse et une consultation d’allergologie, voire d’allergologie-anesthésie, est nécessaire. C’est notamment le cas pour les antécédents de réaction périopératoire non-explorés, associés à un risque réel de récurrence. Dans certains cas (syndrome fruit-latex, allergie/intolérance à la viande de mammifère), l’existence d’une allergie alimentaire peut augmenter le risque de réaction d’hypersensibilité immédiate périopératoire. Il est donc important de rechercher systématiquement ces intolérances/allergies lors de la consultation. Les indications d’un bilan allergologique avant une anesthésie, actuellement bien codifiées, ont fait l’objet de recommandations de la Société française d’anesthésie-réanimation et de la Société française d’allergologie. The pre-anesthetic consultation is a key step in identifying allergic patients. While many of these patients report being allergic to one or more medications, these allergies are not always confirmed. Antibiotics, nonsteroidal anti-inflammatory drugs, and opioids are frequently cited by patients, but they are not necessarily the main causes of perioperative reactions. Some situations are sometimes misinterpreted as allergies (family history, subsequent tolerance to the same drug, non-specific clinical signs), and in these cases, the diagnosis can be easily ruled-out. In the other cases, the history of the disease alone is often insufficient to diagnose a drug allergy, and a specialized consultation with an allergist, or even an allergist-anesthesiologist, is necessary. This is particularly the case for unexplored histories of perioperative reactions, associated with a genuine risk of recurrence. In some cases (fruit-latex syndrome, mammalian meat intolerance), the existence of a food allergy may increase the risk of an immediate perioperative hypersensitivity reaction. It is therefore important to systematically screen for these intolerances/allergies during the consultation. The indications for allergy testing prior to anesthesia, which are now well codified, have been the subject of recommendations by the French Society of Anesthesia and Intensive Care and the French Society of Allergology.
