Integrating Palliative Care Into the Noncommunicable Disease Framework in Low‐ and Middle‐Income Countries

Background Palliative care is a crucial component of comprehensive noncommunicable disease (NCD) management, yet its integration into national health systems remains limited, particularly in low‐ and middle‐income countries (LMICs). Despite the high burden of NCDs in LMICs, including India, palliative care services remain largely unavailable or fragmented. Objective This study explores strategies for integrating palliative care into NCD frameworks within LMICs, with a specific focus on India. It identifies existing gaps, barriers, and opportunities for policy advancement, drawing on global best practices and expert consultations. Methods A focused scoping review of peer‐reviewed and gray literature published in the last 5 years was conducted using PubMed, Scopus, and World Health Organization (WHO) databases. Inclusion criteria encompassed studies describing the integration of palliative care with NCD management in LMICs. Gray literature and policy documents were also reviewed. A total of 35 studies and 12 policy documents met the inclusion criteria. Expert consultations with six leaders in palliative care and health policy were used to refine recommendations. Results Integration models from India (Kerala and Bihar), Uganda, Mexico, South Africa, Malawi, and Bangladesh demonstrate the feasibility of embedding palliative care within primary and community‐based NCD programs. Persistent challenges include limited policy integration, inadequate funding, workforce gaps, and inequitable access. Conclusion Integrating palliative care into national NCD programs is crucial for achieving person‐centered, equitable health systems in LMICs. Implementation should prioritize policy reform, sustained funding, training of generalist and specialist providers, and community engagement. These findings inform public health strategies aligned with Universal Health Coverage and the sustainable development goals (SDGs).
