Transient Cortical Blindness (TCB) Following Transcatheter Aortic Valve Implantation (TAVI): A Case Report

IntroductionTransient Cortical Blindness (TCB) is a rare complication of contrast medium use, occurring in 0.3%–1% of cerebral angiographies and 0.008%–0.05% of coronary angiographies. (1) It presents as partial or complete vision loss despite normal neurological and fundoscopic examinations, with no specific abnormalities on imaging. While contrast agents are generally safe and effective, they can induce TCB in rare cases, which typically resolves spontaneously within 2 hours to 14 days. (2) The underlying mechanism of TCB is thought to involve contrast medium leakage into the vertebrobasilar circulation during catheterization, leading to transient neurotoxicity. Here, we report a case of TCB lasting 12 hours following transcatheter aortic valve implantation (TAVI) for severe aortic stenosis (AS).
