Preparing for International Ophthalmology Fellowships Exams: A Structured Guide for Postgraduates

Abstract Fellowship training is increasingly recognized as essential for ophthalmology postgraduates seeking subspecialization, surgical expertise, and academic advancement. This narrative review consolidates global recommendations from the International Council of Ophthalmology (ICO) and the Royal College of Ophthalmologists (RCOphth), with structured preparation strategies relevant to Indian postgraduate trainees. This review is targeted at Indian ophthalmology postgraduates planning domestic or international fellowships. Recognizing the three-year duration of Indian residency, the review emphasizes early completion of theoretical and standardized examinations (Step 1) within the first two years, followed by clinical and surgical consolidation (Step 2) during the final year. The article outlines domestic and international fellowship pathways, advantages of qualifications, such as FICO and FRCOphth, and their role in facilitating global mobility and General Medical Council (GMC) registration. It highlights the strategic benefits of early planning, research engagement, mentorship, and online educational tools. The integration of academic preparedness, clinical competency, and strategic timing enhances success rates in fellowship acquisition and future career progression.
