Cardiovascular Risk Mitigation During Extended Anesthetic Exposure: Integrating Hemodynamic Monitoring, Myocardial Oxygen Balance, and Perioperative Neurohumoral Regulation A Comprehensive Evidence-Based Review with Clinical Management Framework

ABSTRACT Background: Extended surgical procedures under general anesthesia impose significant hemodynamic stress, autonomic dysregulation, and alterations in myocardial oxygen supply-demand balance. Perioperative cardiovascular complications remain a leading cause of morbidity and mortality in surgical patients worldwide, accounting for approximately 40% of all postoperative deaths. Objective: This review synthesizes current evidence on cardiovascular risk stratification, intraoperative hemodynamic optimization, and neurohumoral regulatory mechanisms in patients undergoing prolonged anesthetic exposure (>3 hours), with the aim of providing an integrated clinical management framework. Methods: A structured narrative review was conducted encompassing 28 primary references sourced from MEDLINE/PubMed, Cochrane Library, and ClinicalTrials.gov databases (2012–2024). Included studies addressed hemodynamic monitoring technologies, myocardial protection strategies, anesthetic pharmacodynamics, and perioperative autonomic modulation. Results: Evidence supports goal-directed hemodynamic therapy (GDHT) as superior to conventional static monitoring for reducing perioperative cardiac events (RR 0.62; 95% CI 0.48–0.80). Maintenance of mean arterial pressure ≥65 mmHg and cardiac index ≥2.2 L/min/m² correlates with reduced acute kidney injury and myocardial injury after non-cardiac surgery (MINS). Neurohumoral dysregulation—particularly catecholamine surge, renin-angiotensin-aldosterone system (RAAS) activation, and natriuretic peptide elevation—independently predicts 30-day cardiovascular events. Conclusions: Integrated perioperative cardiovascular management combining advanced hemodynamic monitoring, individualized anesthetic selection, myocardial oxygen balance optimization, and neurohumoral attenuation significantly reduces the burden of cardiovascular complications in extended surgical procedures.
