Navigating Specialization: A Comparative Analysis in Physical Therapy, Occupational Therapy, Physician Assistants, and Orthotics and Prosthetics

Background: In most medical fields, specializations naturally form mechanisms and organizations to regulate certification of practitioners and maintain clinical standards to protect patients. Currently, specialization in the field of orthotics and prosthetics (O&P) consists primarily of clinicians self-identifying as specialists in particular areas of practice. This study aims to quantify the current mechanisms for specialization within four allied health professional fields: physician assistants (PA), physical therapy (PT), occupational therapy (OT), and O&P. Methods: A scoping review was performed utilizing academic databases to search articles on the pathways of specialization in allied health fields. Additional information on specific specializations and mechanisms involved was obtained from the official websites of each discipline involved (OT, PT, PA, and O&P) as well as associated auxiliary organizations. Results: A total of 119 specializations across four disciplines and their associated mechanisms were evaluated in this study. It was found that 25% of specializations were run through discipline-specific governing boards and 98% through a certification mechanism regulated by a governing body or specialty organizations. Currently, O&P is the only field without any specialties recognized by its governing body and has the least number of specialties overall (two). Conclusions: Throughout the history of medical-based professions, there has been a progression from generalization toward specializations as knowledge and skills have developed. With the growing demand for specializations, similarly the necessity has arisen to implement mechanisms by which to govern has similarly increased. This article reviewed the many different pathways and mechanisms that could be pursued. Clinical Relevance: The findings of this study validate the existence of formal pathways for specialization in similar fields to O&P and the need for formal pathways to specialization to be created in the field of O&P.
