Conclusions: AYA patients with thoracic malignancies constitute a rare and heterogeneous population, frequently presenting with advanced-stage disease and a high prevalence of actionable genomic alterations.Optimal management requires a comprehensive, multidisciplinary approach, integrating molecularly guided therapy with psychosocial support, fertility preservation, survivorship care, and long-term toxicity monitoring.Retrospective design and small sample size are limitations, highlighting the need for multicenter collaborations to improve outcomes and establish dedicated care pathways for this unique patient group.
