From Evidence to Action: Practice-Changing Updates in the 2026 AHA/ASA Acute Ischemic Stroke Guideline

The American Heart Association/American Stroke Association [AHA/ASA] 2026 guideline for early management of acute ischemic stroke1 introduces several recent clinical trials and systems changes that inform practice-relevant updates. Although the fundamental aim of rapid reperfusion has not changed, these new guidelines provide expanded indications for thrombectomy and thrombolysis, new recommendations for prehospital care, first-ever guidance on pediatric care, new imaging and triage paradigms, and supportive care updates. The updated 2026 guideline streamlines acute stroke care by expanding the treatable population and simplifying emergency workflows, but it also highlights ongoing gaps in our knowledge of optimal blood pressure targets and how to manage minor, non-disabling deficits. This article reviews key recommendations that have changed since the 2019 update, emphasizing their clinical and operational impact. Implementation considerations are discussed to support translation of guideline updates into real-world stroke care.
