Inguinal hernia containing the cecum and appendix (Bendavid type 3 sliding hernia): a two-case series

Abstract Introduction Sliding inguinal hernia is an uncommon entity, accounting for approximately 6–7% of groin hernias. Involvement of the cecum and appendix is particularly rare and is frequently missed preoperatively, with diagnosis often made during surgery. Case description We report two cases of elderly male patients with long-standing inguinoscrotal swellings that became irreducible. The first case involved an 88-year-old man who presented with acute irreducibility, obstipation, and vomiting. The second case was a 73-year-old man with a giant inguinoscrotal hernia of more than 10 years’ duration that became irreducible over three months. In both cases, the cecum and appendix were hernia contents and did not form part of the hernia sac wall, corresponding to Bendavid type 3 sliding hernias, these were not classical (‘true’) sliding hernias (Bendavid type 1 sliding hernias) Discussion Inguinal hernias involving the cecum and appendix are rare and predominantly affect elderly males. Their clinical presentation is variable, and preoperative diagnosis remains challenging. Recognition of the sliding component is essential to avoid visceral injury during dissection.
