Doctors to Blame or Not to Blame? Patients to Suffer or Not to Suffer? These Are the Questions

Background: The opioid crisis has promoted aggressive public health interventions aimed at reducing opioids prescription. However, these well-intentioned policies have often neglected the needs of chronic pain patients, resulting in widespread under treatment and increased suffering. At the same time, clinicians face growing scrutiny and blame for contributing to addiction and overdose rates, despite a lack of evidence supporting such claims. Objectives: This article aims to (1) analyze the negative impact of restrictive opioid prescribing policies on chronic pain management; (2) highlight the critical role of mental health factors in overdose and suicide risk; (3) advocate for balanced, patient centered approaches to opioid use; and (4) dispel misconceptions about physician blame in the opioid crisis. Study Design: Narrative literature review and critical commentary. Setting: This work was based on publicly available studies, guidelines, and data from U.S. public health institutions and peer-reviewed journals. Results: Despite some published papers claiming much higher addiction rates among chronic pain patients prescribed opioids, the actual rates can reasonably be estimated at one to three patients per 1000 treated. There is conclusive evidence contradicting narratives claiming that opioid prescribing is the primary driver of the US opioid crisis.
