Beyond willpower: supporting patients through the psychological and physical effects of tobacco cessation

Stopping smoking is associated with psychological and physical symptoms that can feel unexpected and distressing, even when motivation to quit is high. These effects are commonly misinterpreted as personal failure or deterioration in mental health, increasing the risk of relapse. This article outlines the psychological and somatic effects of stopping smoking, with particular focus on nicotine withdrawal and early physiological recovery. It examines what pharmacological aids – including nicotine replacement therapy, varenicline and regulated nicotine vapes – can and cannot address, and emphasises the importance of behavioural support alongside medication. The article also explores how relapse can be framed as part of the learning process rather than treatment failure. By normalising symptoms and setting realistic expectations, general practice nurses can provide effective, compassionate support that improves long-term quit outcomes.
