<h3>Background</h3> By 2040, 63,353 people each year are expected to die with a palliative care need, of which 42% will be aged over 85.<sup>1</sup> Implementing comprehensive strategies for urban and remote rural notably heterogeneous populations, demands thorough Palliative Care Health Needs Assessment across all diagnoses, age groups. Addressing people’s care requirements both in affluent areas with prevalent and deprived areas where more people live alone around the clock, our organization promotes proactive attitudes to prevent care crisis. <h3>Aim</h3> This study aims to evaluate the impact of pre-emptive outgoing calls to patients, carers, and professionals. <h3>Method</h3> Descriptive study analysing the 7883 telephone initiated by organization professionals August 2024 - July 31st, 2025. <h3>Results</h3> 688 patients received support through 7883 telephone interactions: 887 professional-to-professional, 3273 professional-to-patient calls averaging 15 minutes, with 2675 responses from family members and 75 unsuccessful calls, totalling 2676 hours dedicated to outgoing communications from clinicians to patients and their families. While our team received 1857 calls: 1605 seeking advice, 796 from other professionals, 618 from caregivers and family, and 161 from patients. 153 out-of-hours incoming calls were received, initiated by families and caregivers (80), other professionals (45), and patients (28). 153 calls were received Out of Hours both from patients (28), Carer/Family (80) and professional (45), while 1605 Advice Line were attended from patients (191), Carer/Family (618) and professionals (796). In the same period, 580 face to face connections took place (mean duration 85 minutes per encounter). Patients age ranged from 19-99 (mean age 74) <h3>Conclusion</h3> Timely proactive communication is pivotal as connections catalyst among patients, families, and professionals, facilitating Community care during the patient‘s journey.
