PPT16 Improving communication about prognostic uncertainty and illness understanding: a pilot evaluation of the GUIDE E-module

<h3>Background</h3> Clinicians often struggle to promote prognostic awareness and communicate uncertainty in conversations with patients and families. Unfortunately, little formal training exists to support the development of these skills. We developed an e-module using the GUIDE framework (Graph, Understand, Impact, Death, Explore) to provide a structured approach to these skills to facilitate improved advanced care planning (ACP) conversations with seriously ill patients and their families. <h3>Methods</h3> This pilot study explored the experiences of 14 palliative medicine trainees who used the GUIDE e-module in April 2025. The e-module includes didactic content, sample scripts, case-based learning, and a video demonstration. Participants provided qualitative and quantitative feedback through surveys that assessed changes in attitudes, knowledge, confidence, and preparedness for ACP discussions. <h3>Results</h3> All respondents agreed the module met its learning objectives. The GUIDE mnemonic, visual illness trajectories, and video demonstration were consistently valued. Post-module, 93% strongly agreed that discussing illness trajectories is key to patient understanding (up from 71%). Knowledge improved, with 43% reporting ‘superior knowledge’ post-module (vs. 7% pre-module). Confidence in discussing death as a possible outcome rose to 93% (from 64%). Feedback suggested reducing content density and including more complex clinical scenarios. <h3>Conclusions</h3> Data suggests the e-module improved trainees’ readiness to lead ACP conversations that promote prognostic awareness, navigate uncertainty, and support patients in making informed decisions. The structure and visual tools were effective, offering a promising scalable model for enhancing serious illness communication skills. Future iterations will consider enhanced clinical complexity, cultural sensitivity, and increased interactivity to promote learner engagement and content applicability.
