Practical Insights into the Synergistic Effect of the Combination ofIbuprofen and L-Arginine as Rapid Action Therapy in Pain Relief

Effective pain management remains a challenge in clinical practice. Pain is frequently underdiagnosed, and although effective medications exist, patients with pain, especially chronic pain, are often undertreated. Ibuprofen arginate, the salification of free (RS)- ibuprofen with L-arginine, is a widely used over-the-counter (OTC) analgesic and anti-inflammatory medication. The metabolic bioinversion of R-(-) ibuprofen to the active S-(+) ibuprofen is unidirectional and depends on the absorption rate. Therefore, modifying the racemic ibuprofen formulation to enhance the absorption rate of the active S-(+) enantiomer increases the drug’s effectiveness in alleviating pain. This non-steroidal anti-inflammatory drug (NSAID) combines the synergistic effects of ibuprofen and L-arginine, resulting in an increased absorption rate of the active S-(+) enantiomer. Based on this mechanism, rapid onset of analgesia and enhanced anti-inflammatory effects are observed. The addition of L-arginine, a substrate of nitric oxide synthases (NOS), further protects against NSAID-induced gastric mucosa damage. Moreover, inhibition of prostaglandin synthesis in vivo is more potent with S-(+) ibuprofen than with the R-(-) enantiomer. Clinical studies have consistently demonstrated the beneficial effects of ibuprofen arginate in various moderate-to-severe acute pain settings. This article presents an updated narrative review of the rationale and advantages of combining ibuprofen and L-arginine from pharmacokinetic and clinical perspectives in the treatment of patients with moderate-to-severe acute pain of different causes.
