<i>Editorial Commentary</i> : Progression to Osteoarthritis Following Posterior Cruciate Ligament Reconstruction Is Common, With Possible Differences Based on Surgical Technique

Various techniques have been described for posterior cruciate ligament reconstruction with ongoing debate regarding the optimal technique, specifically with regard to single-bundle versus double-bundle reconstruction. While biomechanical data have showed that double-bundle reconstructions more closely restore native knee biomechanics with improved restraint to posterior tibial translation, clinical data remain mixed without definitive superiority. Inherent heterogeneity in injury and patient factors makes identifying optimal techniques challenging. To date, most comparative studies have found small or no clinical differences based on graft type, fixation, or single-bundle versus double-bundle techniques, leaving surgeons to preferentially rely on personal experience, training background, expertise, cost, available resources, and patient-specific needs to render the most appropriate care.
