遺残坐骨動脈瘤に対しExcluder 脚を用い血管内治療を行った1例

A persistent sciatic artery (PSA) is a very rare with an estimated incidence of 0.01-0.06%. PSAs can have associated aneurysm formation 15% to 44% of the time, which can result in thromboembolism, rupture, or pain secondary to permanent parlay due to sciatic nerve compression. There are no guidelines for the treatment of persistent sciatic artery aneurysms, and treatment procedures have not yet been established. In Europe and the United States, the treatment is the aneurysm resection and replacement or the embolization of the aneurysm with bypass surgery, and endovascular treatment of aneurysms with stent grafts. Japan has many challenges to solve for treatment of persistent sciatic artery aneurysm (PSAA), and there is not a single stent graft device that is reimbursed by insurance in Japan. Therefore, there have been very few case reports on the use of endovascular stent-grafts for PSAAs. In this report, we describe a case of endovascular treatment of a PSAA and discuss the literature review.
