Comparison of efficacy of I-Gel and Baska mask in laparoscopic surgery with pneumoperitoneum

Background: Laparoscopic surgery requires effective airway control as intra-abdominal pressure is increased by the pneumoperitoneum (PN), which may compromise breathing. To ensure airway patency, supraglottic airway devices (SADs) such as the I-Gel and Baska mask are used. Less is known about their ability to maintain breathing in the PN during laparoscopic procedures. Aims and Objectives: This study aimed to evaluate the I-Gel and Baska mask for adequacy of ventilation after PN during laparoscopic surgery. Materials and Methods: A total of 70 adult patients undergoing elective laparoscopic surgery participated in this prospective, randomized study. Patients were randomly assigned to the I-Gel or Baska mask groups (n=35). Leak fraction, inspiratory volume (VINSP), expiratory volume (VEXP), and end-tidal carbon dioxide (EtCO2) levels were monitored after mask insertion, before PN, and every 10 min thereafter for up to 70 min. The insertion time of the device, insertion attempts, and problems such as sore throat or bloody secretions were noted. Results: The Baska mask showed significantly lower leakage percentages than the I-Gel up to 60 min after PN (P<0.05). At different time points, the Baska mask showed larger VEXP (P<0.05). VINSP and EtCO2 were similar in both groups. Both devices had similar insertion times, trials, and complications. Conclusion: The Baska mask outperforms the I-Gel mask in terms of sealing and respiratory efficiency during laparoscopic procedures with PN. In clinical use, both devices are safe, effective, and well-tolerated.
