Abstract

われわれは，心筋梗塞後に僧帽弁後尖のtetheringを認めた重症虚血性僧帽弁閉鎖不全症（IMR）の60歳男性の1例を報告する．亜急性期に，自己心膜を用いた弁尖拡大および二次腱索切断を併用した僧帽弁形成術を施行した．術後経過は良好であり，術後5年の心エコーにおいても僧帽弁逆流の再発は認められず，左室機能の改善も得られた．本症例は，左室への直接操作が困難な状況において，弁尖拡大と二次腱索切断を組み合わせた僧帽弁形成術が，亜急性期のIMRに対する有効な外科的選択肢となる可能性を示唆している．
