Rare clinical case of successful bail-out therapy after complete transection of the common bile duct

The paper considers the clinical case of the successful bail-out therapy of the patient with the bilioduodenal fistula after complete transection of the common bile duct. Conventional surgery was contraindicated due to the complex anatomic conditions associated with previous surgery, clinical presentation of cholangitis, and high anaesthesia risk. Stabilization of the condition and compensation of the biliary passage in the first stage were achieved by external-internal drainage through the fistula. Further, the selected tactics of the bail-out mini-invasive treatment proved to be effective in maintaining long-term adequate internal bile flow as preparation for a potential surgical correction.
