Registro de intervenciones en cardiopatías congénitas de la Sociedad Española de Cirugía Cardiovascular y Endovascular: 2024 y retrospectiva de los 13 años previos

La Sociedad Española de Cirugía Cardiovascular y Endovascular (SECCE) presenta el reporte de actividad en pacientes con cardiopatía congénita ( CC ) del periodo 2012-2024, junto al registro general de actividad 2024. Los centros participantes registran sus datos de manera voluntaria y anónima. En 2024 aumenta mínimamente el número de cirugías de CC en España, manteniendo cifras similares a los años pre-pandemia. Presentamos agrupados los datos de actividad de los últimos 13 años, para reflejar mejor nuestros resultados con estas patologías relativamente infrecuentes. En este periodo fueron realizadas 26.067 cirugías de CC, que constituyen el 9,5% de la cirugía cardiovascular mayor de nuestro país durante ese tiempo. Un 82% de las cirugías congénitas se hicieron con circulación extracorpórea (CEC) y el 18% restante sin ella. Destacamos las intervenciones neonatales y en adultos con CC que asocian elevada complejidad, y suponen respectivamente un 16,86% y 21,23% de la actividad total. Las CC operadas más prevalentes continúan siendo los defectos septales (comunicación interauricular e interventricular) en los casos que requieren CEC y en pacientes operados sin CEC los ductus y coartaciones. Los datos se presentan ajustados a la escala Aristóteles básico de riesgo quirúrgico preoperatorio. La mortalidad observada en cirugías con CEC fue 2,96% (Aristóteles-6,49) y en cirugías sin CEC 2,28% (Aristóteles-4,98). Nuestro registro nacional de cirugía en CC consolida sus buenos resultados. Esta herramienta permite establecer comparaciones a nivel nacional e internacional, valorar tendencias temporales, ayuda a establecer estrategias de análisis y mejora la calidad de nuestra asistencia clínica diaria. The Spanish Society of Cardiovascular & Endovascular Surgery reports its activity data in patients with congenital heart disease for the period 2012-2024, based on a voluntary and anonymous registration involving most of Spanish Units. This article is complementary to the 2024 cardiovascular surgery annual report, and they are published together. In 2024 we observe a minimal increase in congenital cardiovascular surgery, reaching scores similar to pre-pandemic years. We included data from the previous 13 years, in order to obtain real information related to our activity with these relatively scarce pathologies. From years 2012 to 2024, 26067 congenital heart surgeries were performed, accounting for 9.5% of all major surgery (congenital + acquired) performed in Spain during that period. Of these surgeries, 82% of them required extracorporeal circulation and 18% not. We highlight the operations in neonates and adult patients, which account respectively 16,86% and 21,23% of our whole activity, and are a real challenge. The most prevalent congenital heart pathologies operated on continue to be: atrial/ventricular septal defects in cases requiring extracorporeal circulation, and ductus/coarctation in patients without extracorporeal circulation. The presented data are adjusted to the basic Aristotle score of preoperative surgical risk. The observed mortality of surgeries with extracorporeal circulation was 2.96% (Aristotle-6.49), and without cardiopulmonary bypass 2.28% (Aristotle-4.98). Our national registry of surgical activity in congenital heart disease maintains good results. This tool allows for comparison within an national and international framework along time, help us to establish analysis strategies and improve the quality of our daily practice.
